Create and manage forms from Slack
Add the ChatterForms bot to a Slack workspace. Create, list, and delete forms from a DM or /chatterforms. The bot does not notify the channel when someone submits.
Slack is not a HIPAA destination. Do not use the bot to operate HIPAA forms.
How it works
- Click Add to Slack and approve the workspace install.
- After you sign in, create a live API key on the API keys page.
- Paste the key into the Slack setup screen, or run /chatterforms setup.
- DM the bot or use /chatterforms to create, list, or delete forms.
FAQ
Does Slack tell me when someone submits a form?
The Slack bot does not. To notify Slack when someone submits, use Zapier (beta) (New Submission → Slack). HIPAA forms cannot use Slack or Zapier.
Can I use this with a HIPAA form?
No. Treat Slack as a non-HIPAA surface. Use the ChatterForms workspace for HIPAA forms.

After you have an account, create a live key on the API page. Public API docs are on /apidocs.
